Our Work

The Palliative Partnership of the Roanoke Valley has been a catalyst for many initiatives whose aim has been to promote dignified and respectful end of life care. This page summarizes some of the work that the PCPRV has accomplished.

Clergy Coalition

Members of our clergy represent a trusted source of counsel to many who face complex, serious and potentially lifelimiting illness.

The Palliative Care Partnership of the Roanoke Valley recognizes the impact that our faith based leaders may have in advance care planning conversations and is dedicated to providing members of this community with the tools and education to support them in this worthwhile work.

As such, quarterly trainings are held covering a variety of topics at no charge to any who wish to participate in the Roanoke Valley and surrounding communities.

Patient and Family Council

By virtue of its design, the healthcare system is complex and, at times, confusing to navigate. Patients and family members often have to advocate for resources needed.

The Palliative Care Partnership of the Roanoke Valley has developed a document designed to help patients and families compare and contrast the type and levels of care that may be presented as options in a treatment journey.

Advance Care Planning

The Palliative Care Partnership of the Roanoke Valley has been the pioneer in advance care planning training and education in the communities we serve. Advance Care Planning Facilitators are trained utilizing evidence based, Person Centered Framework.

Two didactic concepts merge to form an approach to end of life care that results in knowing and honoring the wishes of those we love:

 

  • Advance Care Planning which is preparing people for future medical decisions and
  • Shared Decision Making which is preparing people with serious illness in making current healthcare decisions.

 

The training provided by the Palliative Partnership targets health care professionals and equips them with the knowledge and skill necessary to engage others in meaningful end of life conversations.

POST - Physicians Orders for Scope of Treatment

Individual wishes and values form our end of life plans. Documenting those wishes and values increases the likelihood that those wishes will be honored in a medical care setting. The Palliative Partnership of the Roanoke Valley recognized the need a decade ago to adopt a process whereby those wishes can be translated into a physician’s order that is recognized and honored across all settings of care. The PCPRV initiated work with the National POLST Paradigm to bring this very important process to healthcare settings in the Roanoke Valley and surrounding communities.

POLST - National POLST Paradigm

The POST form provides a framework for care for providers to put in place orders that ensure that seriously ill patients with life limiting illnesses or advanced frailty receive the treatments they want and avoid the treatments they don’t want.

Stories from the Heart: Opening the Door to End-of-Life-Conversations

Featuring Donald Davis, Storyteller

On November 7, 2022 the Palliative Care Partnership of Roanoke Valley hosted “Stories from the Heart: Opening the Door to End-of-Life Conversations” with Donald Davis at the Grandin Theatre in Roanoke, Virginia. Approximately 150 people attended the event that featured three stories followed by a panel discussion with local end-of-life care experts.
